In order for the Zaritsky Law Firm to begin working on your claim as soon as possible, we must gather the available information in your possession. Consequently, the Zaritsky Law Firm recommends that you bring any potentially relevant information concerning your claim to the initial consultation.
Importantly, the Zaritsky Law Firm will be able to assist in obtaining critical items that are not in your possession. Depending on your type of case, the claim related information may include the following:
PERSONAL INJURY CLAIM
- Photographs of vehicles involved and/or damaged in your accident;
- Photographs of your accident scene (especially if you were injured on the property of another);
- Photographs of visible injuries (e.g., lacerations/cuts, scrapes, bruises, staples/stitches);
- Accident / Incident / Police Report;
- Basic information (contact/insurance) exchanged with other motorists;
- Basic information (contact) exchanged with potential witnesses;
- Names and descriptions of local businesses/employees that may have witnessed the accident;
- Any statements (e.g., eye witnesses, first responders/emergency personnel, police officers/law enforcement)
- Basic information (contact) for treatment providers (medical/doctors/chiropractors/other) before (5 years) and after the accident;
- Basic information (contact) for treatment facilities before (5 years) and after the accident;
- Insurance policy (or policies), declaration(s), application(s), card(s), and brochure(s);
- Any correspondence from insurance companies or others involved in the accident;
- Any documents showing the types and amount of insurance at the time of the accident;
- Any health insurance policies (HMO’s, PPO’s, HAS’s, other) or other treatment related documents;
- Any insurance claim numbers;
- Basic information (contact) for any insurance claims personnel/representatives/adjustors that have been assigned to your claim or attempted to contact you;
- Notes and/or Diary at the time of and after the accident;
PROPERTY DAMAGE LOSS
- Photographs / videos / inspection reports of property damaged;
- Photographs of your accident scene;
- Accident / Incident / Police / Fire Marshall Report;
- Basic information (contact) for potential witnesses;
- Any statements (recorded, written, or otherwise);
- Homeowner/Commercial Insurance policy (or policies), declaration(s), application(s), card(s), and brochure(s);
- Any correspondence from insurance companies or others involved in the loss;
- Any documents showing the types and amount of insurance at the time of the loss;
- Any insurance claim numbers;
- Basic information (contact) for any insurance claims personnel/representatives/adjustors that have been assigned to your claim or attempted to contact you;
- Proof of payments made / received concerning the loss; and,
